What to bring

Referral from your General Practitioner or Specialist Physician

A referral provided by your General Practitioner will last for a period of 12 months unless your doctor specifies “ongoing” or “indefinite” referral. After 12 months it is essential for you to provide a new referral from your General Practitioner to meet Medicare and Health Insurance Commission (HIC) requirements. If you have been referred by a Specialist Physician or through a public hospital your referral will only last for a period of three months. Please contact Nepean Cardiology if you are unsure of your referral status.

Completed Patient Information Sheet

If you’re a new patient you will receive a New Patient Information Sheet with your appointment confirmation letter. You can also complete this form on the day of your first appointment or you can submit one online here.

Medications

Please bring a complete list of your current medications or, if you’d prefer, please bring your packages of current medication.

Recent Results

Please bring results of any recent pathology, X-ray or CT scans.

Cardiac Investigations Referral Slip

If you are an existing patient of Nepean Cardiology and you have been referred to have an outpatient Cardiac Investigation please ensure you bring your Referral Slip showing the appointment date and time as this slip shows the reason for the investigation and is required by our Cardiac Technician.

Confirmation of Third Party Cover

If your appointment relates to a Workcover or Transport Accident Commission (TAC) claim and you would like Nepean Cardiology to invoice your Consultation or Investigation expenses directly to the relevant company, you will need to supply written confirmation supporting your claim.
